Registration

We now have two ways for you to register.

Option 1:
Download and fill out our Registration form. You can either drop it off at the office or mail it along with the $50.00 registration fee to:

Option 2:
Go to our Online Registration page, complete the form and
send the file. Please drop off or send the $50.00 registration fee to:

Children will be placed the first week of January. We place children on a first-come first-serve basis. Present and past parents are
given priority.

A confirmation or waiting list letter will follow.

IMPORTANT

The State of Minnesota requires that you complete the following forms and submit them to Bethany Nursery School by Aug 1st.

Child Care Immunization Record
Health Care Summary (must be completed by health care source)
